At The Crossroads reaches out to homeless youth and young adults at their point of need, and works with them to build healthy and fulfilling lives. They focus on young people who are not otherwise accessing services. They accomplish their mission through a combination of nighttime street outreach, 1-to-1 counseling, and work with collaborators who provide opportunities for their clients. At The Crossroads aims to build healthy trusting relationships with their clients, through which they can identify goals and develop plans to accomplish them. They work with other agencies to ensure that there is an accessible, comfortable continuum of support that will provide their clients with realistic opportunities to achieve their goals.

Personal Bio
In 1997, with an idea bolstered from our work on our undergraduate theses, Rob Gitin and I received echoing green fellowships to start At The Crossroads, a street youth support program. We provided direct services to street youth and young adults, and through that work, I became interested in health and healthcare delivery to adolescents and young adults. After 4 years, I went on to pursue an MD, and during medical school worked with the Adolescent Health Working Group to expand public insurance funding in San Francisco to cover young adults to age 25 within 300% of poverty level. I pursued residency in combined Internal Medicine and Pediatrics (Med-Peds) in Minneapolis, and since completion, joined the faculty at University of Minnesota. I am in the process of establishing a care system for youth and young adults with special health care needs who are transitioning from Pediatrics to Adult Medicine.
